Treatment is stage-specific. In the freezing phase we focus on pain management with TENS, ultrasound, and gentle mobilisation. In the frozen stage, progressive joint mobilisation, capsular stretching, and Codman pendulum exercises are used. In the thawing phase, strengthening and functional rehabilitation are prioritised.

Questions

Everything you need to know before your first session of Muscle Stiffness. Our experts are always available to help.

Frozen shoulder causes progressively worsening pain and stiffness in the shoulder, with all movements (especially rotation and lifting) becoming restricted. If you cannot lift your arm past shoulder height and washing your back is impossible, frozen shoulder is likely.

Without treatment, frozen shoulder can last 1–3 years. With physiotherapy, recovery time is significantly reduced. Most patients achieve good recovery in 6–12 months with consistent treatment.

Treatment is adjusted to your pain levels. In the freezing stage, we focus on gentle techniques. Some discomfort during stretching in the frozen stage is normal and expected, but treatment should never be excessively painful.

Yes — the vast majority of frozen shoulders (over 90%) resolve with physiotherapy without needing surgical manipulation under anaesthesia. Surgery is only considered after failure of adequate conservative treatment.

The exact cause is unknown in many cases. Risk factors include diabetes (3–4x higher risk), thyroid disease, Parkinson's disease, previous shoulder injury or surgery, and prolonged immobilisation after injury.

Night pain is characteristic of frozen shoulder and is caused by inflammation within the joint capsule. We provide specific sleep position advice to minimise night pain significantly.

In the freezing phase: pendulum exercises and gentle active ROM. In the frozen phase: pulleys, wand exercises, and progressive stretching. We guide you precisely based on your current stage.

Most patients regain full or near-full shoulder function after frozen shoulder with proper physiotherapy. A small percentage may have some residual stiffness, but this rarely causes significant functional limitations.
